Woodside Dog Boarding
DOG BOARDING WAIVER AND RELEASE OF LIABILITY

1.       Health and Vaccinations

I confirm that my dog is in good health, free from contagious diseases, and up to date on required vaccinations (including rabies, DHPP, and Bordetella unless otherwise agreed in writing). I understand that failure to provide proof of vaccination may result in my dog being denied boarding services.

2.       Medical Emergencies

In the event of a medical emergency, Woodside Dog Boarding will make every reasonable effort to contact me. If I am unreachable, I authorize Woodside Dog Boarding to seek veterinary care for my dog. I understand that I am financially responsible for all veterinary costs incurred.

3.       Behavioral Issues

I certify that my dog has not shown aggressive behavior toward humans or other dogs. I understand that if my dog displays behavior deemed dangerous or disruptive, Woodside Dog Boarding may isolate them or contact me to arrange early pickup.

4.       Injury or Illness

While every effort will be made to ensure the safety and well-being of all dogs in care, I understand that accidents, injuries, or illness can occur. I release Woodside Dog Boarding, its staff, and representatives from any liability related to injury, illness, or death of my dog while in their care.

5.       Personal Items

I understand that personal items (beds, toys, leashes, etc.) may not be returned in the same condition or may be lost or damaged. I will not hold Woodside Dog Boarding responsible for any such occurrences.

6.       Fleas and Ticks

If fleas or ticks are discovered on my dog, I authorize treatment at my expense. I agree to keep my dog on a regular flea/tick prevention program.

7.       Payments and Cancellations

I agree to pay all fees in full at the time of pickup unless otherwise agreed. I understand that cancellations made less than 24 hours before the scheduled boarding may incur a cancellation fee.

Acknowledgement

I have read, understood, and agree to the terms above. I release Woodside Dog Boarding, its owners and employees any and all liability for injury, illness, or damages related to my dog’s stay.
